Surrey Podiatry Service – Self-referral
Provides diagnosis and treatment for problems with your feet and lower limbs in a variety of locations that includes local health Centres, acute hospitals, community hospitals, GP practices and patients homes.
How do I refer into the service?
The podiatry service is open access which means that they accept self-referrals as well as referrals from health professionals and other agencies.
Referrals can be made by completing the Virgin Self Referral form July 2018 and posting to the central centre:

To enable referrals to be triaged and processed as quickly as possible, we ask you to provide the following information:
- NHS number if known
- details of the presenting problem
- Details of past and present medical history
The referral process:
Referral received > Referral triaged > waiting list > Appointment generated and booked at local clinic.
